Media Release Form for Minor Athletes

I, being the parent/guardian of the athlete(s) identified below, hereby consent that the photographs or videos (collectively “media”) taken of him/her/them during the course of any sporting event in 2024-2025 may be used by Relive the Moment LLC (“Relive the Moment”) and others working for Relive the Moment on its behalf.

This media may be used in marketing material, on the Relive the Moment company website, on any social media platform, or in any printed or digital media.

While I may receive digital and/or physical copies of the media, I agree that I have no financial claim in any of the media. I shall have no right of approval, no claim to compensation, and no claim to invasion of privacy arising out of any use. I give Relive the Moment the right and permission to use my child’s image, likeness, and voice in any future media, including media created or sold for commercial use.
